At CJRG, completing the required forms ensures we have the information needed to provide safe, efficient, and compliant healthcare services. These forms typically include patient registration, consent and authorization, medical history, insurance information, and any program-specific documentation. Accurate and timely submission helps us deliver care without delays while protecting your privacy and maintaining compliance with healthcare regulations. All forms can be submitted electronically or in person at our facility.
Please consult with your health carte providers office on which docuemnts you will need to provide to them. These forms must be submitted and reviewed by your health care providers office before you are elected for surgey.
Mandatory Forms
These forms will be required upon arrival to your assessment. These can be filled out and printed at home or physical copies are avaiable on visit to your health care provider.

Language Assistance
All forms and documentation must be presented in English. If you need support in filling out forms or documents, Shared Health Manitoba has 24/7 support available through your health care provider. Please be sure to let your health care provider know prior to filling out these forms that you will will need assistance, as they will be the ones to get you in contact with your interperter.
